WAIVER AND RELEASE OF LIABILITY
In consideration of being permitted to participate in activities organized by So-Melo Grinders, a Travel Team with players aged 15 and under, I, the undersigned, agree as follows:
Assumption of Risk: I understand that participating in sports activities involves inherent risks, including but not limited to the risk of injury, illness, or death. I voluntarily assume all risks associated with my participation in So-Melo Grinders activities, including those risks arising from the actions or negligence of others.
Release and Waiver: I hereby release, discharge, and hold harmless So-Melo Grinders, its directors, officers, employees, agents, volunteers, and representatives from any and all claims, demands, causes of action, liabilities, damages, or expenses (including attorney fees) arising out of or related to my participation in So-Melo Grinders activities, including but not limited to any injury, illness, or damage to person or property.
Indemnification: I agree to indemnify and hold harmless So-Melo Grinders, its directors, officers, employees, agents, volunteers, and representatives from any and all liabilities, damages, or expenses (including attorney fees) incurred as a result of my actions or omissions during my participation in So-Melo Grinders activities.
Medical Treatment: In the event of an injury or illness, I authorize So-Melo Grinders and its representatives to obtain medical treatment for me, including but not limited to emergency medical care, hospitalization, or surgery, as deemed necessary. I understand that I am responsible for any costs associated with such medical treatment.
Photo and Video Release: I consent to the use of my likeness in photographs, videos, or other media taken during So-Melo Grinders activities for promotional or educational purposes without compensation.
Acknowledgment of Understanding: I acknowledge that I have read this waiver and release of liability, fully understand its terms, and voluntarily agree to be bound by them. I further acknowledge that this agreement shall be binding upon myself, my heirs, executors, administrators, and assigns.
